Most of the following forms may be filled out electronically, but will need to be printed and then mailed, faxed, or scanned/emailed back to AMIDEAST. *Note: you will NOT be able to save your responses in the file electronically.

These forms should be filled out and returned to AMIDEAST as soon as possible after arriving in the United States – before October 9th.
